Mechanism of Action (MOA): It is a selective for anaerobic bacteria due to their ability to intracellularly reduce imidazole which covalently binds to DNA ,disrupt its helical structure,inhibiting bacterial nucleic acid synthesis

Indications & Dosage

Trichomonas Vaginitis
Adult :2g taken as a single dose,taken preferably before meals
Children: 30mg/kg as a single dose.

Acute Intestinal amoebiasis
Adult: 2g taken as a single dose,taken preferably before meals
Children: 30mg/kg as a single dose.

Giardiasis
Adult: 2g taken as a single dose,taken preferably before meals
Children: 30mg/kg as a single dose.

Severe Invasive amoebiasis
Adult: 1.5 g daily as a single dose or in divided doses for 5 days,preferably just before meals
Children: 30mg/kg for 5 days

Hepatic Amoebiasis
Adult: 1.5 g daily as a single dose or in divided doses for 5 days,preferably just before meals
Children: 30mg/kg for 5 days

Contraindications

Secnidazole is contraindicated in

  • Patients with hypersensitivity to imidazole derivatives
  • During pregnancy and Lactation
  • Patients with blood dyscrasias

Warning and Precautions

  • Caution should be exercised in patients with history of blood disorder.
  • Avoid alcohol consumption while taking this medication ( Because of possibility of Disulfiram effect)

Care should be taken in the following Conditions:

  • Hepatic impairment
  • Alcoholic cirrhosis
  • In obstructive Liver diseases liver disease
  • Use cautiously with other hepatotoxic drugs and visual field changes
  • Avoid alcohol use during therapy
  • Neurological disorders

Pregnancy and Lactation:

Secnidazole should not be administered during the first trimester of pregnancy or during lactation because it is found in placenta and breast milk

Adverse Effects:

The clinical studies have shown that secnidazole is well tolerated and no serious adverse reactions have been reported to the date.
The following side effects may be observed with secnidazole as with all nitroimidazole derivatives and are rarery serious:

Nausea
Epigastric pain
Metallic taste
Glossitis
Stomatitis

Occasional
Urticaria
Moderate Leucopenia which is reversible on treatment discontinuation

Rare
Vertigo
Ataxia and motor incordination
Paresthesia and Peripheral Neuropathy

Very Rare
Nausea
Vomiting
Epigastric Pain

Drug Interactions:

Anticoagulants:
May potentiate anticoagulant effect of Warfarin and increased risk of haemmorrhage ,so simultaneously it requires close monitoring during treatment

Disulfiram
Antabuse-like effects,confusional state and paranoid reactions may occur

Cimetidine
Half life of Secnidazole may be prolonged

Lithium
Plasma levels of lithium increased leading to toxicity

Overdosage

At therapeutic doses Secnidazole is well tolerated ,however if an overdose should keep watch on the patient,and presenting Nausea ,vomiting ( or other toxicity data) ,is taken to hospital and general measures of support
Treatment is supportive and symptomatic.Drug is removed by induced emesis,Gastric lavage and administration of activated charcoal & use of carthartics.
For controlling of seizures diazepam & Phenytoin may be used.
